CHAP

We will guide your preparation for the Community Health Accreditation Program (CHAP). CHAP accreditation is for community-based health care organizations, such as homecare, hospice and
durable medical equipment vendors that are recognized for excellence in community health practice.
CHAP has been granted deeming authority by the Centers for Medicare and Medicaid Services for homecare, hospice and durable medical equipment organizations.
PSN can provide expert CHAP consulting services for CHAP accreditation preparation in any of the following areas:
•  Review and/or assist in writing policies and procedures compliant with the appropriate
CHAP standards
•  Make on-site visits to perform a Gap Assessment and develop a plan of corrective action
•  Provide a continuous plan for monitoring compliance with CHAP standards
•  Assist with completion of the CHAP application and self-assessment activities
•  Review job descriptions, contract elements, forms and other information necessary to
demonstrate compliance to the CHAP standards
•  Offer leadership training on various accrediting agenda topics such as Medication Management,
the use of Data Management, QI/Performance Improvement, Infection Control or Strategic Planning
•  Provide consultation to support development of programs required by CHAP, including performance improvement with tailored databases to measure compliance to CHAP standards, patient safety, emergency preparedness/management and infection control
•  Assist with the implementation of the Evidence Based Practices for homecare, as required by OASIS-C, effective January 1, 2010
